Cultural Competency touches individuals, families and workers who care for others within the circles of LGBTQ+ people. Thousands are dying earlier than their straight counterparts because they are isolated and afraid to ask for help.

But a growing number of people are fighting to keep their elders from being silenced. This is a moving and informational symposium which will provide awareness of the unique struggles facing the LGBTQ community. It will help you navigate to a place of greater understanding and compassion for this population.

The symposium will offer 6.5 credit hours of continuing education for professionals and a certificate of completion for family caregivers.  

Sponsors of this event will be promoted to over 5000 individuals on the web, social media and through sponsors and media partners.  Attendees are professional persons in caregiving roles: they are conscious consumers with significant purchasing power who support sponsors of their training.

2020 CULTURAL COMPETENCY SYMPOSIUM SPONSORSHIP COMMITMENT FORM

Cultural Competency Symposoim (“Symposium”) to be held on Friday, January 24 2020 at the Lovers Lane United Methodist Church, 9200 Inwood Rd, Dallas, TX.

The procurement of sponsorships is to set the event-related expenses, to enhance the participant experience, to promote The Sponsor’s commitment to the care-giving community and to publicize The Sponsor’s product or services. Ed-U-CARE, Inc. reserves the right to approve or disapprove any potential sponsor.

The advantages and benefits of sponsorship are outlined in the attached Sponsorship Levels document. The Sponsor has selected the following level of support.

 

PLEASE SELECT ONE LEVEL:

The Sponsor will contribute the amount selected above to Ed-U-CARE, Inc. (2020 Symposium) for the sponsorship at the time of commitment. 

Ed-U-CARE, Inc. reserves the rights to any intellectual property developed for and/or created as a result of the Symposium. No recording, photographing, videotaping or broadcasting of the Symposium is permitted without the written consent of Ed-U-Care, Inc.

In addition, the Sponsor agrees to not share booth space at the Symposium with persons who are not in contract with Ed-U-Care, Inc. for this sponsorship. This includes displaying promotional collateral materials for companies other than the Sponsor.

LIABILITY RELEASE FORM

1. In consideration of being permitted to participate in the Compassion Fatigue Symposium scheduled to be held on January 24, 2020 (the “Event”), I hereby release, waive, discharge and agree not to sue Ed-U-Care, Inc., the participants, speakers, instructors, sponsors, advertisers, owners, lessors and lessees of the premises and each of them and their directors, managers, members, shareholders, officers, agents, employees, representatives and attorneys (the “Releases”) from any and all liability for injury and damages of any kind and any and all cost and expenses for any act, action, occurrence, omission, circumstance regarding, concerning, arising out of and/or occurring in or about the Event including by reason of the NEGLIGENCE OF THE RELEASEES.

 

2. I understand that participation in the Event could result in injury, bodily harm, property damage and/or other damages. I hereby assume any risk of harm, injury or damages, which might occur to the me due to my participation in the Event.

 

3. I grant and convey to Ed-U-Care, Inc. all rights, title and interest in any photographs, images, video, or audio recordings of me, my likeness or voice made at the Event.

 

I HAVE READ THIS LIABILITY RELEASE AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. I AM SIGNING THIS ELECTRONICALLY AND I INTEND AND AGREE BY TYPING MY NAME BELOW I AM SIGNING THIS AGREEMENT AS THOUGH I HAVE PUT MY PEN TO HAND AND SIGNED THEREBY.
